What this website collects
When you send an appointment request or a message, we receive the details you type, such as your name, phone number, email address and the days that suit you. We use them only to reply and to schedule your visit. We never sell your information.
This site uses privacy-friendly analytics that count visits without cookies and without identifying you personally. We don’t use advertising pixels, heat maps or session recording on this site.
Please don’t send health information through this site
This website’s forms and email aren’t built to protect health information. Please don’t include symptoms, medical conditions, medicines, birth dates, government ID numbers or insurance ID numbers in a form or an email. We’ll ask for what we need by phone, in person or through our practice software, which is built for it.
Reviews on this site
Reviews quoted here are the reviewers’ own public words, with their names as they posted them. We never reply to a review by confirming someone is a patient or discussing their care.
Other companies’ tools
Some pages show a map. Those tools have their own privacy policies and only load when you use them.
Your choices
You can ask us to see, correct or delete what you’ve sent through this website. Contact us using the details at the bottom of this page.
Notice of Privacy Practices (HIPAA)
Brightside Family Dental is a health care provider covered by the federal HIPAA Privacy Rule. This is a summary of our Notice of Privacy Practices. You get the full notice at your first visit, and you can ask us for a copy any time at the front desk or by phone.
How we may use and share your health information
We use and share it to treat you, to bill your insurance or you, and to run our practice (for example, to coordinate with a specialist we refer you to). We share it for other reasons only when the law allows or requires it, or when you sign an authorization, which you may take back at any time.
Your rights
You can see and get a copy of your records, ask us to correct them, ask for a list of who we’ve shared them with, ask us to limit how we use or share them, and ask us to contact you in a different way, such as at a different phone number. You can ask for a paper or electronic copy of this notice.
Complaints
If you believe your privacy rights have been violated, you can complain to us or to the U.S. Department of Health and Human Services. We won’t retaliate against you for complaining.
Who to ask
Our privacy officer is Renee Kowalski. Call (585) 555-0142 or write to hello@brightsidefamilydental.com. Please don’t include health details in an email.
